That's the crazy thing about blows to the head. It doesn't have to be a hard hit, as it can be any sort of hit that will ring that bell. Also, helmet or not, a pro hockey player coming in on skates probably doesn't require much force to cause a brain injury. We not talking fluffy stay puft...
A hit to the head is a hit to the head. You just never know the severity of the hit until sometimes it's too late.
If I ran the NHL, all hits to the head, regardless of intent, would be called a penalty. Review the play if necessary and reverse the call if warranted. You want to stamp out head...
