It has to do with them literally looking like they're hygienic and pristine. Like they smell good. Like they shower and don't do gross things, such as the time I saw Spears on a late night talk show picking between her toes for gunk and flicking it off. If you know what to look for, you can tell when someone is dirty and when someone is clean.
